B2OSH XXIII Registration Instructions

Please fill in the form from your computer. It’s an interactive, fill-in, .pdf form. You may need to download the latest version of Acrobat Reader. Typing data means we don’t have to decipher chicken scratches. There are a couple data fields with more than one line. Note: Data entry will not scroll to the next line. If you need both lines, hit <ENTER> or <TAB>.

Enter numbers into the order part of the form. The form does the arithmetic for you. Some computer OS/software combos do not allow saving the form with data, but everyone should be able to print it after filling in the blanks. If you can save it after inputting data, then you can e-mail it to us an attachment. But, we can only accept checks via snail mail. Snail mail the completed form along with a paper check. We are unable to handle electronic funds transfers, such as PayPal or credit card charges or direct deposits.

Please print out the completed form, write your check, and mail to the address on the form.

Also, please e-mail the completed form to: LarryG@b2osh.org. You should be able to save the completed form. If not, then please print, scan, & attach to an e-mail.

There is uncertainty in these difficult financial times regarding sponsorship. We will again cover the $20 ramp fee in Rockford if our finances permit.

Pilot & Passengers
Note: To reduce confusion, we will NOT use the term “Co-pilot”. For B2SOH purposes “Co-pilot” = “First Adult Passenger”. It doesn’t matter if your passengers are pilots, certificated or not.

Please indicate T-shirt sizes along with names. The ONLY choices are:
Adult S, M, L, XL, 2XL, 3XL, 4XL and Youth YXS, YS, YM, YL

Listing sizes does NOT mean you are ordering or buying anything. Cygnet Aerospace is sponsoring Flight Shirts again this year. Everyone in Rockford will get one, hence the need for sizes. Wear your Flight Ts Saturday so your B2OSH shirt is clean for our team photo at the party on Sunday.

Please list children’s ages. B2OSH participation by kids is strongly encouraged. “Demographic” data or a “Kid Census” will help us create cool “stuff” and cool ways to keep their interest.

Contact Info
E-mail address and cell phone numbers are the important things here. This is how we communicate before RFD and after, respectively. We rarely need your cell number, but when we do, it’s important. Forgot to pay for gas at RFD? Emery can get your cell number from us. One year an aircraft taxiing into our camping area banged a wingtip into of our plane’s prop & nose bowl. Quick call to the “damage-ee’s” cell number, and the aircraft owner was notified ASAP.

B2OSH & Formation Experience
On the Years line, type in the most recent years you’ve flown B2OSH until you run out of space. On the Formation Experience lines, cite military training, B2OSH clinic attendance, & other noteworthy formation training & experience.

Party Attendance
We need head counts for our parties. We assume everyone will be at both the RFD Hangar Party & the B2OSH Party. A few folks do not arrive RFD till Saturday AM and miss the Hangar Party. Some folks register, skip RFD, and meet us in OSH. We need to know who those people are, so we can order the correct amount of food.

Order Form
Note: 1st passenger is $25. This is a change. All passengers are $25. This is an increase.

“Kid” = under 18. Eligible voters are NOT kids. We considered changing the cut-off age to 13 because teenagers eat like Hungry Smead (free Margarita for anyone who knows that obscure reference w/o looking it up). But, we noted that teenagers are the future of aviation and decided we can afford to feed some for a few more years.

Late Registration
Late Registration is for plane, not each person. Leave blank if on time; enter “1” if late. If you enter “2” or something higher, the form will dutifully do the math and you will overcharge yourself. Thanks for the donation!

Shirts & Sizes
This year’s B2OSH polo shirt is 100% Peruvian Pima cotton, in a pique knit. They are high quality garments. Solid color, no pockets, B2OSH logo embroidered on left breast. Available in Small, Medium, Large, XL, 2XL, 3XL, 4XL, 5XL, and 6XL.

There is a Women’s line. Women’s sizes WS, WM, WL, WXL, W2XL and W3XL.

What about kids who are too small for these shirts? The past few years we’ve embroidered Ts for the little kids. Last year we found a reasonably priced kids’ polo. We plan to provide shirts, free of charge, to kids who are too small for the ones you can buy. That’s another reason we need sizes on the passenger listing. Note: If your child fits into an Adult Small (or larger) shirt, we hope you will buy a B2OSH shirt.

To order clothing, enter the numbers of each garments & sizes in the appropriate boxes. The form does the arithmetic. It may be slow to update. Be patient. If you change your mind, totals will not self-correct until you TAB to the next box. If you goof, enter “0” or the correct number, then TAB or ENTER and wait for the form to do its magic. If it doesn’t work, add it by hand and LET US KNOW!

Replacement Name Tags
We cut a name tag for every first-time participant. You’re supposed to keep the tag for the next time. If you lose it, we will cut another one for $5. Below cost. It’s a bargain. Please type in the name you want on the badge. Exactly as you want it cut.
