more legible, please download by clicking on the appropriate link below.
Each form has fill in blanks to be filled in on your computer
before printing. Please comply rather than hand printing
the forms. This makes Larry's job and that of the clinic sponsor
much easier.
Pay attention: Every year we get
about ten people over-paying their registration. Your base
registration fee covers you and one additional adult whether it be a
pilot, spouse or friend. All children less than 18 years old are
free. If you insist on paying an extra ten dollars for that one
additional adult or child we will consider it a contribution and we thank you in
advance.
There is uncertainty in these
difficult financial times regarding sponsorship. We will again
attempt to cover the full ramp fee with the 50.00 registration if our
finances permit.
